Synopsis of All Hell Breaks Loose

The Charmed Ones are trying to protect an innocent doctor from an assassin demon named Shax who has been sent by the Source. They bring the doctor back to the house, and Phoebe goes up to the attic to look in the Book of Shadows to find a spell to vanquish the wind demon, Shax. Prue and Piper start to explain to the doctor a little bit of what is going on, but then Prue feels a chill. Shax busts in the front doors. Prue calls upstairs to Phoebe and Phoebe yells back that she's almost ready. Shax attacks the doctor and Prue pushes him out of the way, taking the blow herself. She is blown back through a wall and falls unconscious to the floor. Piper tries to attack, but Shax blows her through a wall as well. Shax is about to kill off the doctor when Phoebe runs downstairs and recites the spell she found. Shax goes poof. Phoebe calls for Leo who orbs in and heals Prue and Piper, who were close to death, lying bleeding on the floor unconscious. Phoebe says she thinks she just injured Shax, didn't kill him. They suspect you need more than one witch to recite the spell to give it the strength it needs. Prue and Piper go out with the spell to try to vanquish Shax for good, while Phoebe stays with the doctor at the house.
Piper and Prue find Shax on the street, or rather, Shax finds them. Before they can use the spell, Piper uses her exploding power to defend herself and is pleased when Shax goes poof. Prue thinks that the poof wasn't quite a vanquishing poof, and that they might not have gotten rid of him for good. A live news broadcast catches the sisters in action.
Back at the house, the sisters explain to the doctor why he has to keep their secret, and he happily agrees and thanks them for saving his life. After he leaves, Prue asks Leo to make sure that they really vanquished Shax, 'cause she thinks they didn't. Phoebe has also been researching how to save Cole and reverse what evil spell Raynor cast on him. She asks for her sisters' help in sending her down to the underworld using a modified "magic to magic" spell so that she can find Cole and save him with a potion she made. Her sisters are reluctant, but do so.
In the underworld, Phoebe faces Cole. He threatens her, but she uses the potion and they embrace and kiss. She tries to convince him to come back home with her, but he fears that they won't be able to make it work up there.
Back at home, Prue and Piper find out that they're all over the news. Reporters, wanna-be witches, and crazy people are all over their front lawn. Darryl comes over to try and help control the backlash. Leo confirms that they didn't actually vanquish Shax, so they sneak out of their house and drive to the hospital to find the innocent doctor.
They grab the doctor and take him out to their car to keep him safe. Reporters once again catch Shax attacking and the sisters successfully vanquishing him this time using the spell and reciting it together.
Prue and Piper head back home and try to figure out what to do. Leo says that the powers that be are really pissed, and the only idea they have is to beg the demon Tempus to turn back time. Meanwhile, one of the wanna-be witches busts in and asks to join their coven. An impatient Prue uses her power to throw the loopy girl back out the door. The sisters ask Leo to go down to the underworld, find Phoebe and Cole, and ask Cole to use his demon connections to get Tempus to turn back time.
In the underworld, Cole tells Phoebe she needs to go home, 'cause something big is going down and the Source just arrived. Leo orbs in and tells Cole what they need. Cole says he doesn't have the power to find Tempus. Leo says that the Source does, and if Cole pitches the idea to the Source as his, to protect the demon world from the exposure it has gotten as well, the Source might do it. Cole goes to the Source, but the Source knows what's in Cole's heart and says he'll get Tempus to do it if Phoebe agrees to stay in the underworld. Cole says Phoebe would never agree to that. The Source says Phoebe will agree to save one of her sisters' lives.
At home, the wacky wanna-be witch is upset that she was rejected by Prue and Piper, and climbs on top of her van and shoots into the house. She gets Piper in the gut. Prue freaks out and takes a heavily bleeding Piper out to the car. She puts Piper in the back seat and tries to drive away to the hospital, but the crowd is too thick and she can't get through. Prue freaks out, 'cause her sister is dying in the back seat and no one will get out of the way. She finally uses her power to throw a bunch of people out of the way, and the rest scatter. She drives away to the hospital.
Even with immediate care from the innocent doctor, Piper dies at the hospital. Meanwhile, Prue has been classified as criminally dangerous and is being hunted by militia while trying to mourn her sister's death.
In the underworld, Cole comes back to Phoebe and Leo and tells them what the Source said. Leo orbs back up to see if the Source was telling the truth and finds Prue barricaded in a hospital room with Piper's body. Leo orbs back down, and Phoebe can immediately see from his face that Piper is dead. Phoebe agrees to the Source's demands. Since time won't go back in the underworld, only in the real world, Phoebe asks Cole to go and warn Prue and Piper as soon as the deed is done.
Cole goes to the Source and says it's a deal. The Source dismisses Cole and tells one of his many minions to hold Cole and kill Phoebe as soon as time goes back. Time goes back.
Prue and Piper start to explain to the doctor a little bit of what is going on, but then Prue feels a chill. Shax busts in the front doors. Prue calls upstairs to Phoebe but Phoebe doesn't answer. Shax attacks the doctor and Prue pushes him out of the way, taking the blow herself. She is blown back through a wall and falls unconscious to the floor. Piper tries to attack, but Shax blows her through a wall as well. Shax then turns on the doctor and blows him through a window. Shax looks around at the wreckage with pleasure and then blows back out through the front doors of the house, shattering the windows in the doors as he slams them behind him. Prue and Piper are left lying in pools of their own blood.
